The Referral Process

To refer a patient to Smoky Mountain Sports Medicine & Physical Therapy, call our dedicated referral line: (828) 550-3923.  This is a central scheduling number and staff will be able to schedule your patient at any of our locations. Please fax the PT order, including patient name, date of birth, and contact phone number, and any other information to (828) 354-0209.  We will schedule your patient quickly and efficiently. Following our evaluation, we will fax your office our PT Treatment Plan of Care for your review.
